• Primary Physician needs to issue a completed
FL2 – giving the facility complete orders for treatment
TB Skin test or Chest X-ray showing free of communicable diseases
Copies of the last few doctor's office visit notes
Copies of any last hospitalizations Discharge Summary
We will also need to obtain copies
Power of Attorney
Health Care Power of Attorney
Living Will
Social Security Card
Medicare Card
Medicaid Card
Secondary Insurance Cards
AARP, BCBS
Medicare Part D
If a Resident has applied for financial assistance: You need to go to the local Social Service office explaining the need to apply for Special Assistance Medicaid
If a Resident is covering the financial cost of care themselves: the Administrator or designee will discuss the needs of your loved one and review the cost of care.
