How to Admit a Resident to Our Community...

• Complete the admission application

• Primary Physician needs to issue a completed

  • FL2 – giving the facility complete orders for treatment

  • TB Skin test or Chest X-ray showing free of communicable diseases

  • Copies of the last few doctor's office visit notes

  • Copies of any last hospitalizations Discharge Summary

We will also need to obtain copies

  • Power of Attorney

  • Health Care Power of Attorney

  • Living Will

  • Social Security Card

  • Medicare Card

  • Medicaid Card

  • Secondary Insurance Cards

  • AARP, BCBS

  • Medicare Part D

If a Resident has applied for financial assistance: You need to go to the local Social Service office explaining the need to apply for Special Assistance Medicaid

If a Resident is covering the financial cost of care themselves: the Administrator or designee will discuss the needs of your loved one and review the cost of care.
